A combination of two or more can be accepted as identification: birth certificate, marriage certificate, medical card, driving licence, passport, paid utility bills, bank or building 4 society cards or statements, National Insurance number card, payslip, letter from Benefits Agency, papers from Home Office

After the medical examination is complete, you will be issued with a certificate that can be used to obtain or renew a private hire or hackney carriage licence with the city of Wolverhampton council.
