Your GP surgery can also provide your medical summary. This is free service and under GDPR rules they are not allowed to charge you for this. Contact your GP surgery to obtain your records.

A combination of two or more can be accepted as identification: birth certificate, marriage certificate, medical card, driving licence, passport, paid utility bills, bank or building 4 society cards or statements, National Insurance number card, payslip, letter from Benefits Agency, papers from Home Office

After the medical examination is complete, you will be issued with a certificate that can be used to obtain or renew a private hire or hackney carriage licence with Wigan council.
